How to Get Started

Information that will need to be gathered and reviewed by MEORC:
  • Copy of check stubs or pay information from the most recent 6 pays and dates of employment
  • Copy of health insurance, Medicaid and Medicare cards, if applicable
  • Verification and amounts of:
    • Social Security benefits, your social security number and date of birth
    • Any recent notices from Social Security
    • Any housing subsidy (official letter of amount)
    • Food stamps (letter from the local Department of Jobs and Family Services)
    • Any assets (car, house, bank account or other)
  • If there are any expenses related to disability that are paid to enable work, they could be deductible from earned income, to determine countable income. The most common is expense for medication/supplements. If this is applicable, we will need copies of the receipts for the past 3-6 months.
